It’s a clip that has understandably drawn significant outrage from social media users as it went viral this week.
The footage, reportedly captured by a hidden camera placed in room by his wife, appears to show Gulf War Marine veteran Alfred O’Toole being shoved, pushed, pulled, and hit by an employee of the facility where he received care for serious neurological issues.

This is a disgrace.
A Gulf War Marine veteran named Albert O’Toole who served this country, took a blast that left him with TBI, and was later diagnosed with Alzheimer’s was beaten by his aide Matthew Cox inside the New York State Veterans Home in Montrose, New York.
His wife Angela Sangro became concerned after seeing unexplained bruises and heavy sedation. She installed a hidden camera. The footage shows the aide yanking food out of Albert’s hands, grabbing him by the neck, throwing him into a chair, punching him, and striking him in the head with a broom while he cried out in pain. No one came to help.
The state fired the abuser.
But the federal VA? Still has him employed. Still paying him while the case drags on.
You make veterans fight through hell just to get the benefits and compensation they earned with their blood and sacrifice. You move fast to cut or reduce what’s owed to them.
But when one of your own is caught on camera assaulting a defenseless veteran? You keep him on the federal payroll like it’s nothing.

His challenge to VA Secretary Doug Collins did not go unanswered.
In his own post, he promised action, making the important distinction that the facility in question is not operated by his agency:

New Jersey’s News 12 also highlighted the shocking footage in a recent article:
The Turn To Tara team’s investigation found that the problems may be more widespread than just her husband’s case.
Data found in the investigation revealed that the state-run home racked up more than 47 complaints and 25 citations between 2021 and 2025.
This included deficiencies in involving restraints, medications and failures to report suspected abuse, resulting in warnings and staff terminations.
The home was also hit with more than $29,000 in fines in April 2025, issued through Medicare after a state inspection cited failures to prevent and report alleged abuse.For Sangro, the findings are both deeply troubling and personal.
She says her husband fought for his country, and now she is fighting for him.
“I never dreamt that this would be our life,” she says. “I can’t even sleep anymore. I just watch him at night on that camera to make sure nobody’s hurting him.”
A spokesperson for Veterans Affairs sent a statement to News 12 about the situation.
“VA is aware of the allegations against Mr. Cox related to a non-VA facility. VA takes these allegations seriously and has removed this employee from VA patient care duties while it awaits the adjudication of his arrest,” the spokesperson wrote.
Tiegen acknowledged Collins’ response in a follow-up post:

This only happened this fast because people refused to stay silent. The sharing, the tagging, the pressure it’s working. Movement is being forced.
But “initiate proceedings” is not the same as getting it done. We’ve seen how these things can drag on for months while someone still collects a check.
Secretary Collins @DeptVetAffairs finish it. Cut the pay. Complete the removal. Permanently bar this man from any work with veterans. And do it fast.
This case also exposed a bigger problem. The Montrose home already had 47 complaints and 25 citations for abuse-related failures. Current oversight failed. That’s why there must be a full independent third-party investigation into all New York State Veterans Homes.
We don’t stop here.
We don’t move on.
We stay on this until the abuser is gone, until real accountability happens, and until these homes are properly investigated and fixed.
